Thecaloscopy Reduces the Risk of Recurrent Perineural (Tarlov) Cysts after Microsurgical Resection

Retrospective study evaluated whether combining thecaloscopy (endoscopic arachnoid sheath fenestration) with traditional microsurgical resection reduces the recurrence of symptomatic sacral Tarlov cysts in 78 patients. The authors found that adding thecaloscopy significantly lowered cyst recurrence to 10.4% compared to 30% for those receiving microsurgical resection alone, while maintaining a low complication rate.
